In the theory of medical science, at its foundation, the idea is that the brain just magically comes alive and decides “You know what? The left ear can sound different then the right, if a person pays attention they can sense which direction the communication is coming from. They can sense the left ear, the right ear, a space on the upper right corner, right directly above the mind space centered at the top and outside of the head all together.
